ÖSSZEFOGLALÁS:
Szerzők a tonsilla alsó pólusánál elhelyezkedő peritonsillaris tályog esetét mutatják be, amely a musculus constrictor pharyngison áttörve parapharyngealis tályogot is okozott. A parenterális antibiotikumterápia és incízió nem hozta meg a várt javulást, ezért CT-vizsgálatot követően abscess-tonsillectomia történt. Így sikerült biztosítani a tályog kiürülését és a gyógyulást. A szerzők a nemzetközi irodalommal összevetve levonják az eset tanulságait: az elkezdett konzervatív terápiára, incízióra nem javuló esetekben gyanakodni kell a tonsilla alsó pólusa mögötti tályogra, esetleg parapharyngealiss tályogra. Ilyen esetben jó megoldás lehet az abscess-tonsillectomia. A diagnózis felállításában a CT-vizsgálat segíthet.
Case of a parapharyngeal abscess sprung from the peritonsillar space with review of the literature.
SUMMARY
The authors present a case of a peritonsillar abscess that broke through the musculus constrictor pharyngis and caused a concomitant parapharyngeal abscess. The result of the parenteral antibiotic therapy and incision was not satisfying, so after CT imagination an abscess tonsillectomy was performed. This way the drainage of both abscesses was resolved and resulted complete recovery. The authors draw the conclusion of this case comparing with literature: when the conservative therapy and incision does not lead to recovery, we should suspect a possibly abscess behind the inferior pole of the tonsil or a parapharyngeal abscess in which diagnose the CT imaging is utile and in these cases abscess tonsillectomy can be useful.
