ÖSSZEFOGLALÁS:
A parotistumorok sebészeti kezelésében mindig a megfelelő radikalitás és az arcideg védelme volt kulcskérdés, emiatt számos operatív szemlélet és műtéti típus alakult ki. Jelen tanulmánnyal eredményeink tükrében hangsúlyozzuk a műtéti típusok ismertetése mellett az ún. extrakapszuláris disszekció létjogosultságát. Osztályunkon 2014. 3. 1. és 2019. 4. 1. közti időszakban 79 parotissebészeti beavatkozást végeztünk. A daganat mérete és elhelyezkedése miatt biztonsággal 28 esetben tudtunk extrakapszuláris disszekciót alkalmazni. Az extrakapszuláris disszekciók során idegsérülés, mint szövődmény nem fordult elő. Eredményeink és a hivatkozott irodalom alapján minden operatőrt bíztatni tudunk a parotis hátsó-alsó pólusában elhelyezkedő, fizikális vizsgálattal és aspirációs citológiával benignusnak véleményezhető terimék extrakapszuláris disszekcióval történő eltávolítására. Az arcideg feltárásának elkerüléséhez idegmonitor hiányában nagy segítséget ad az Ikoma-féle tumorlokalizációs beosztás. Az eljárás további előnye, hogy esetleges reoperációnál a korábbi műtét okozta hegesedés nem növeli a n. facialis sérülés kockázatát.
Extracapsular dissection of parotid tumors: our experiences with the introduction of a new approach
SUMMARY
Proper removal of tumors and protection of the facial nerve have always been key issues in the surgical treatment of parotid tumors, which has led to the development of a number of operational approaches and types of surgery. In the present study, in addition to the description of the types of surgery, we emphasize the edxtracapsular dissection. In our department between I. 3. 2014. and I. 4. 2019 we performed 79 parotid surgical interventions. Due to the size an location of the tumor, we were able to safely use extracapsular dissection in 28 cases. There was no nerve injury in the cases extracapsular dissections. Based on our results and the cited literature, we encourage all surgeons to perform extracapsular tumor removal if it is consideredbenign by physical examination and aspiration cytology, which are located in the posterior and lower parotid gland areas. In the absence of a nerve monitor, the Ikoma Tumor Localization Classification is of great help in avoiding facial nerve injury. The procedure further has the advantage that, in case of a reoperation, scarring caused by previous surgery would not endanger the facial nerve.
