ÖSSZEFOGLALÁS
Az osseointegrált implantátum rendszerek több évtizede bizonyítottan jól alkalmazhatók a külső- és a középfül eredetű veze- téses halláscsökkenések, továbbá az indikációs terület bővülésével a féloldali siketség rehabilitációjában. Az eltérő Baha rendszerek azonban mind az alkalmazott műtéti technikában, mind pedig az implantátum+abutment egység fizikai paramétereiben, környezethez való viszonyukban jelentősen különböznek. Anyagok és módszerek: A Baha DermaLock implantációs rendszert, nemzetközi megjelenésével egy időben, 2012 októberében vezettük be klinikánkon. Az új rendszer különbözőségeit mutatjuk be első két esetünk ismertetésével, részletes áttekintést adva az implantátum+abutment egység újdonságairól, továbbá a többszörösen módosított műtéttechnikáról.
Eredmények: A Baha DermaLock implantációs rendszer alkalmazásával a műtéti technika leegyszerűsödött, a beavatkozás időtartama ezáltal jelentősen, az eredeti technika harmadára- felére csökkent. A műtéti beavatkozás szinte kivétel nélkül helyi érzéstelenítésben, a lágyrészek maximális mértékű megkímé- lése mellett végezhető. A posztoperatív sebgyógyulás gyorsabb és tökéletesebb, így a késői szövődmények incidenciája elmarad a korábbi technikák esetén tapasztaltaktól. A posztoperatív audiometriai eredmények mind a hallásküszöb, mind a beszédértés szignifikáns mértékű javulását mutatják.
First experience with DermaLock Baha implant system.
SUMMARY
Introduction: The osseointegrated implant systems are shown to be well applicable for several decades in the rehabilitation of conductive type hearing losses with outer-, and middle ear origin, and furthermore along with the widening of the indication field in single sided deafness as well. Distinct Baha systems however differ significantly from each other in the applied surgical methods, and in the physical parameters of the implant+ abutment unit, and in its relation to the environment.
Methods and materials: At the same time of the international appearance of Baha DermaLock implant system in 2012 October, we introduced it into our clinical practice, too.
Based on the two first subjects we interpret the differences of the new system, by giving thorough overview about the novelties of the implant+abutment unit, and also about the multiply modified surgical methods.
Results: Parallel to the use of the Baha DermaLock implant system the surgical technique became simpler and thus the surgical time reduced significantly, to the third-half of the original method.
Apart from some exceptions the intervention could be performed in local anaesthesia, with the maximal preservation of the soft tissues. The postoperative wound healing is quicker and more perfect, thus the incidence of late complications are lower than those found in the case of prior techniques. The postoperative audiometric results showed significant improvement in hearing thresholds and speech recognition as well.
