ÖSSZEFOGLALÁS:
Tanulmányunkban egy 53 éves nőbeteg esetét ismertetjük, akinél a progresszív nyelési nehezített- ség és a nyakba sugárzó neuralgiform fájdalom hátterében egy a bal oldali parapharyngealis teret és a fossa pterygopa- latinát is érintő pleomorph adenoma igazolódott. A fizikális vizsgálat során egy sima felszínű, ép nyálkahártyával borított, submucosus térimét találtunk, amely a bal oldali lateralis garatfalat és a tonsilla árkot antero-medialis irányba diszlokálta. A mágneses rezonancia (MRI) vizsgálat megerősítette a bal oldali parapharyngealis térfoglalás klinikai diagnózisát. A transoralis aspiratiós cytológiai vizsgálat (FNAC) egy benignus, kisnyálmirigy-eredetű tumor lehetőségét vetette fel. A tumor elhelyezkedésének és potenciálisan benignus voltának megfelelően, transoralis enoscoposan-támogatott behatolást választottunk. A tumor en bloc resectióját követően a szövettani vizsgálat pleomorph adenomát véleményezett. Két héttel később, elsődleges sebgyógyulás mellett a beteg korábbi tünetei megszűntek. Összefoglalva, a parapharyngealis tér elsődleges pleomorph adenomája rendkívül ritka. A sikeres kezelés alapját a megalapozott preoperatív diagnózis és a teljes tumor eltávolítás jelenti.
Summary:
We report a rare case of pleomorphic adenoma involving the parapharyngeal space and pterygopalatine fossa. The case report presents a fifty-three-year-old woman who was referred with dysphagia and neuralgiform pain. Clinical examination revealed a smooth submucosal mass in the oral cavity, which diplaced the lateral pharyngeal wall and tonsillar fossa antero-medially. Magnetic resona- ce (MRI) scan confirmed the diagnosis of left-sided parapharyngeal neoplasm. Transoral fine needle aspiration cytology (FNAC) suspected the cytological diagnosis of a benign tumour originated from minor salivary glands. According to the tumour location and benign nature, we decided transoral approach combined with endoscopic assi- tance. The tumour was en bloc resected and the histo- pathological examination confirmed the diagnosis of pleomorphic adenoma. After 2 weeks, a primary wound healing was found and the patient reported complete recovery and vanishing symptoms. In conclusion, primary pleomorphic adenoma arising in the parapharyngeal space shows an extreme rare occurance. The clinical suspicion and the correct tumour resection with a cuff of surrounding normal tissues are the most important circumstances of sue- cesful treatment.
