ÖSSZEFOGLALÁS:
A szerzők olyan beteg esetét ismertetik, akinek önakasztás után leszakadt a légcsöve a gégéről. A beteget sürgős légcsőmetszés után utalták osztályukra. Felvétele során a légcsőnyílás fölött látható volt, hogy a gyűrűporc és a leszakadt légcső között több centiméteres foly- tonossághiány van. Ezen a területen szembeötlött a nyelőcső előboltosulása. A hangszalagok indirekt vizsgálat alapján paramedián állásban helyezkedtek el. Sebkimetszés után end to end anasztomózissal állították helyre a gége és a légcső folytonosságát. A sebgyógyulás után néhány hónappal később a bal oldali hangszalag részleges laseres ary- tenoidectómiával és submucosus chordectomiával kombinált laterofixációját végezték, majd a beteget dekanülál- ták. Az elvégzett műtétek és a tracheostoma záródása óta öt év telt el. A beteg azóta is jól kap levegőt, hangja kissé gyengébb a normálisnál, de jól érthető.
SUMMARY
The authors describe the case of a patient whose trachea has been separated off from the larynx after self-hanging. The patient undervent urgent tracheotomy and transmitted to the department of the authors. In the course of admission it has been seen that a gap of continuity of several centimetres was present between the cricoid cartilage and the broken-off trachea. In this area the vaulting of the trachea was conspicuous. Based on indirect examination, the vocal cords were in paramedian position. After excision of the wound edges the continuity of the larynx and trachea was restored by end to end anastomosis. After healing of the wound, some months later partial arytenoidectomy and submucosal cordectomy was performed with laser, preserving and lateralizing the medial mucous part of the vocal cord according to Lichtenberger. Than the patient was decannulated. Five years have passed since the performed operations and the closing of the tracheostoma.The breathing of the patient is satisfactory, the voice is slightly weaker than normal, but can be well accepted for every day conversation. This case represents, that also combined laryngotracheal injuries could be managed with satisfactory result performing adequate techniques in appropriate time.
