ÖSSZEFOGLALÁS:

Szerzők beszámolnak a kiterjedt szupraglottikus gégedaganatok endoszkópos laseres kezeléséről tracheotomia nélkül. Az elmúlt 5 év során 22 beteget operáltak a Steiner, illetve Rudert által kialakított műtéti eljárások és elvek szerint. Tumor- klasszifikáció szerint 2 Tl-es, 16 T2-es és 4 T3-as szupraglottikus daganatos beteget operáltak. Műtét során az epiglottist valamennyi esetben vertikálisan felezték és a gége szupraglottikus részét cranio-caudális irányba haladva több részletben távolították el. 22 betegük közül 19 él és recidívától mentes. Három beteg halt meg: egy beteg helyi recidívában, egy beteg a nyaki blokk területén kialakult recidiva és egy beteg májcarcinoma miatt. Az onkochirurgiai eredmények megítélésére még hosszabb betegkövetés szükséges, a funkcionális eredmények viszont összehasonlíthatatlanul jobbak, mint amelyek a klasszikus szupraglottikus gégereszekciókkal elérhetők. A módszer nagy előnye, hogy tracheotomiára nincs szükség és az esetek döntő részében tápszonda alkalmazására sem.

SUMMARY

The authors report on endoscopic laser management of supraglottic laryngeal cancer without tracheostomy. In the last five years they have operated on 22 patients with methods and principles pioneered by Steiner and Rudert. The stages of the patients were according to the TNM classification: two Tl, sixteen T2 and four T3, who were suffering from supraglottic cancer. In all the patients the epiglottis was divided by laser in a vertical direction and the supraglottic part of the larynx was removed in several parts in the cranio-caudal direction. 19 of the 22 patients recovered and are in a good condition without any tumour recurrence. 3 patients died, one of them due to a local recurrence, one due to a tumour recurrence on the neck section site and one patient due to a secondary primary tumour of the liver.
To analyse the onco-surgical results of these patients a longer follow-up is necessary. However it can be stated that the functional results are far better than could have been achieved by the classic supraglottic laryngectomy. A great advantage of this method is that it has tendered a tracheostomy superfluous and also the feeding tube need not be applied in most of the cases.
