ÖSSZEFOGLALÁS:

Szerzők ismertetik az 5'aminolevulinsav (ALA) felületi (nyálkahártya) alkalmazásakor indukált protoporphyrin IX fluoreszcencia lehetőségét a garat- és gégedaganatok endoszkópos diagnosztikájában. Az ALA hatására a daganatokban (praecancerosis) szelektív protoporphyrin IX akkumuláció jön létre, amely indukciós fénnyel megvilágítva, speciális endoszkóppal vizsgálva vörös színben fluoreszkál. Ugyanekkor az egészséges szövetek zöld autofluoreszcenciát mutatnak, így jól elkülöníthetők egymástól. A garat- és gégetumorokban a nyálkahártya felületére bevitt ALA (öblögetés, inhaláció), megfelelő inkubációs idő (1,5 óra) után éri el csúcskoncentrációját a tumorszövetben, ekkor végezhető el a vizsgálat. Kezdeti tapasztalataik alapján az ALA fluoreszcens diagnosztika segítséget jelent a daganatok kiterjedésének, határainak pontosabb megítélésében garat- és gégetumorokban, használható tumor excisio utáni sebágy intra- és posztoperatív ellenőrzésére. 

SUMMARY

The authors familiarize us with the possibility of protoporphyrin IX fluorescence induced by the application of 5 aminolevulic acid (ALA) on the surface of the mucous membrane in the endoscopic diagnosis of pharyngeal and laryngeal tumours. The effect of ALA in the tumours (praecancerosis) comes into being through the selective accumulation of protoporphyrin IX. This can only be seen, if we illuminate it with induction light and examine it under a special endoscope. Than it shows up in a red fluorescent light. At the same time the healthy tissues show green autofluorescence so the two can easily be differentiated. In pharyngeal and laryngeal tumours the ALA is taken on the surface of the mucous membrane by a process of rinsing or inhalation after an optimum incubation time of 1,5 hours. When it has reached its peak concentration in the tumour tissue, examination can be carried out. On the basis of initial experience ALA fluorescence diagnosis can help to determine more accurately the extension and boundaries of the pharyngeal and laryngeal tumours and can be used for an intra- and postoperative check of the wound after tumour excision.
