ÖSSZEFOGLALÁS:

A KTP lézer középfül sebészetben való alkalm azása az elmúlt másfél évtizedben széleskörben elfogadottá vált. A KTP lézer fizikai tulajdonságai révén pigmentált szövetekben és hemoglobinban jól abszorbeálódik, vágásra, vaporizációra és koagulációra egyaránt használható. Közleményükben a szerzó'k ism ertetik az elm últ két év során KTP lézerrel asszisztált tym panoplasticák során szerzett tapasztalataikat, részletezik a lézer alkalmazásának különféle lehetőségeit. A KTP lézer a hullámhosszából (532 nm) következően a látható fény (zöld szín) kategóriájába tartozó, flexibilis száloptika segítségével a m űtéti területre juttatható sugárzás. Alkalmazásával kitűnő haem ostasis biztosítható, a hallócsontláncolat körüli, illetve a középfülben lévő hypertrophisált nyálkahártya vagy granulációs szövet a hallócsontok érintése, azaz cochlearis traum a okozása nélkül vaporizálható. A középfülben lévő összenövések és a korábban behelyezett implantátumok atraumatikusan vághatok, a stapes superstructura eltávolítását követően a teljes cholesteatoma m atrix eltávolítása könnyebbé tehető, s a m űtét folyamán végig kiváló haemostasis biztosítható. A cholesteatoma m átrixának eltávolítását követően a m űtéti terület lézerezésével a residuális cholesteatomák száma csökkenthető. A KTP lézer ugyan meglehetősen drága, rendszeres szervizt és speciális tréninget igénylő eszköz, viszont egy intézményen belül több szakterület (urológia, szemészet, nőgyógyászat, plasztikai sebészet) is eredményesen használhatja, s így hoszútávon megtérülő befektetés lehet.

SUMMARY

The application of the KTP laser in chronic ear surgery became widely accepted over the last fifteen years. Due to this la se r’s performance characteristics the beam is well absorbed in haemoglobin and pigmented tissues, and it has turned out to be an excellent tool to cut, vaporise and coagulate during tym panoplasties. Two years experience with the KTP laser is described by the authors, summarising th e ir experiences gained during KTP laser assisted tympanoplasties. Several applications are reviewed. The 532 nm wavelength of the KTP laser enables it to be delivered 45. ÉVFOLYAM 1999. 4. 2 4 7 FÜL-ORR- GÉGEGYÓGYÁSZAT through a flexible fiberoptic device and the surgeon can hand control the green visible laser beam in a microscopic field. By means of the KTP laser excellent haemostasis can be achieved and the hypertrophic mucous membrane and the granulation tissue inside the middle ear can be vaporised without touching the ossicular chain, therefore cochlear traum a can be avoided. The KTP laser has been effective in cutting through scar tissue, in atraumatically removing previously inserted allographs or other middle ear implants, and in safely disarticulating a mobile stapes superstructure for complete cholesteatoma removal. The KTP laser enables the surgeon to obtain a more effective disease removal in a bloodless field. To embark on KTP laser middle ear surgery special training needs to be completed. Although the equipment is rather expensive and requires a regular service, many departm ents (urology, ophthalmology, obstetrics & gynaecology, plastic surgery) within the same hospital can share it’s use for the benefit of patients.
