Akusztikus rhinometria. Értéke és helye a klinikumban

ÖSSZEFOGLALÁS:

Az akusztikus rhinom etria új, minimálisan invazív, gyors, fájdalmatlan, jól reprodukálható, objektív rhinológiai vizsgáló módszer, amely az elmúlt évtizedben mind a klinikai gyakorlatban, mind a tudományos v'zsgálatok terén széles körű alkalmazást nyert. Segítségével megbízható információk nyerhetők az orrüreg elülső, kritikusan rezisztív szegmenséről: a dimenziók és a lumen nagysága számszerűen leolvashatók, illetve grafikusan megjeleníthetők, a mucovascularis térfogat pedig köbcentiméterben kifejezhető. Az eljárás igen hasznosnak bizonyult a minimálisan invazív endonazális m űtétek ind ik álásáb an és tervezésében, valam int a gyógyszeres terápia nyomonkövetésében. A szerzők 20 betegben összesen 120 pre- és posztoperatív mérést végeztek: septum deviáció esetén septum korrekció, krónikus polyposus sinusitisben FESS m űtét történt. Az MCA (minimal cross-sectional area) a septum, a térfogatértékek pedig a polypectomiával is járó FESS m űtétek eredményességének követésére alkalmasak elsősorban. A szűk orrfélben az MCA 0,29 (±0,18)-ről 0,65 (±0,23) cm2-re növekedett orrsövényműtét után, a térfogatértékek a 0-5 cm-es lumenszegmensben FESS m űtét előtt és után 24,94 illetve 69,31 cm3 (177,9%-os javulás, p<0,001) voltak.

SUMMARY

Acoustical rhinometry is a new, minimal invasive, quick, painless, well reproducable, objectiv rhinological examination method, which has in the last decade both in the clinical practice and in scientific researches gathered wide-ranging ground application. With its help, there can be obtained correct information about frontal, critical resistive segment of nasal cavity: dimensions and, extension of the lumen can be read numerically and, graphically and, the mucovascular volume, however, can be expressible in cubic centimeters. This procedure proved to be very useful in indication and, planning of minimal invasive endonasal operations, as well as in follow up of medicine therapy. The authors have done altogether 120 pre-, and postoperative measurements in 20 patients: in case of septal deviation there has been done septal correction, in case of polypous sinusitis, they performed FESS operation. The MCA (minimal cross-sectional area) is suitable for follow up of septum, the volume datas, nevertheless, go together with FESS-operations of polypectomy and, their effectiveness,in the first place. In the narrow nasal half the MCA increased from 0.29 (±0.18) to 0.65 ±0.23) cm2 after nasal septum operation, the volume values, although, before and, after FESS operation were 24.94 respectively 69.31 cm3 in the lumensegment of between 0-5 cm. (Impromevent was 177.9%,p 0.001).
