ÖSSZEFOGLALÁS:

A szerző az inter-ary területen lévő hegek kezelését és a commissura posteriorban kialakult hegek általa kifejlesztett endo-extralaryngealis varrat-technikára és tűátnyo- mó műszerére épülő műtéti megoldásait írja le.
Az EMG-vizsgálat után a hegeket endoszkópos úton szétválasztják. Az újrahegesedés és összenövés megelőzésére a commissura posteriorban két megoldást alkalmaznak. Amennyiben a hegek kiterjedése 5 mm-nél nem nagyobb, úgy azok szétválasztása után mindkét hangszalagot átmeneti időre lateralizálják. így akadályozzák meg, hogy a hegfelszínek ismét érintkezzenek és összenőjenek. Az előzőnél nagyobb, de a 10 mm-t meg nem haladó hegek esetén lágy, a gégelumen alakját követő szilikon tértartót rögzítenek a gége űrterében lévő heges részek közé a szerző fent említett technikája és műszere segítségével. A lateralizáló fonalakat, illetve a tértartót a műtét utáni negyedik héten távolítják el. A műtétek eredményeként a gége lumene ismét megfelelő tágasságú, és a normális légzéshez és hangképzéshez alkalmas. Fenti eljárásokat sikeresen alkalmazták 11 betegen.

 

Endoscopical microsurgical solution of cicatrical adhesions in the inter-ary region and in the posterior commissura.

SUMMARY

The author writes about his operative methods concerning the solution of cicatrical adhesions in the inter-ary region and, in the posterior commissura.
The operations are based upon the endo-extralaryngeal stitching method and needle-pressing apparatus developed by the author. The adhesions are separated after an EMG examination by endoscopical route. To prevent newer cicatrizations and adhesions in the posterior commissura, they adopt two procedures. If the extension of cicatrices is not bigger than 5 mm, so after separation, both vocal-chords are to be lateralized for a temporary time. So they prevent contacting and growing together the cicatrical surfaces again.
In cases of bigger, than previously mentioned, but not exceeding 10 mm extended cicatrices, the author fixes a soft, the shape of glottis tracking silicone space-holding material between the cicatrical parts being in the space of larynx, with the above already mentioned technique and instrument developed by the author. The lateralizing threads and space-holding material are removed on the fourth week after operation.
As a result of the operation, the space of larynx is appropriate wide and, is suitable to normal breathing and sound-formation. The author has applied this procedures in 11 patients with success.
