ÖSSZEFOGLALÁS:
A szerző beszámol arról, hogy a hemilaryngektomiák után esetenként előforduló, a lument beszűkítő heges összenövések profilaxisa céljából a reszekálí gégefél helyére, az izomperi- chondrium lebeny belső felszínére 3 esetben Thiersch-lebenyt varrtak. Ugyanezen célból 6 betegnél rugalmas Bioplast lemezt alkalmaztak. A műtétet lényegesen leegyszerűsítette, hogy a szűk anatómiai viszonyok miatt nehézkes varrat technika helyett a Bioplast-lemezt fibrinragasztó segítségével rögzítették az izom-perichondrium lebeny belső felszínére. A gégebemenet kialakításában előnyösnek bizonyult a reszekált oldalon craniáüsan megkímélt - a műtét radikalitását nem veszélyeztető - porcszegély. A lument beszűkítő heges összenövést, zsugorodást a több éves kontroll idővel rendelkező betegeiknél sem láttak. Tapasztalataik alapján a hámfosztott területek bontására a Bioplast 4-5 hetes felszívódási időre előkészített formáját ajánlják.
The usage of Thiersch-flaps, Bioplast and fibrin-glue in hemilaryn- gectomies.
SUMMARY
The author deals with 3 hemilaryngestomy cases, where he has sewed Thiersch-flaps into the removed laryngeal half upon the inner side of the myoperichondrial flap, because of prevention of lumen-narrowing strictures.
For the same reason they applied elastic Bioplast plates in 6 patients. The operation was simplified essentially through this procedure, because, instead of difficult sewing in narrow anatomical circumstances, the Bioplast plate could be fixed to the inner surface of the myoperichondrial flap by the help of fibrin-glue.
In forming the entrance of the larynx, the spared chondral edge cranially placed of the resection-side was seen to be advantageous and, the radically of operation is not jeopardized. In controlled patients there was not seen, even after years, any knitting or contraction, which would narrow the lumen.
In their experience, to cover the lacks of epidermis, they found Bioplast the best, in form of to be absorbed within 4-5 weeks. The procedure is recommanded.
