A gyermekeken végzett hallásjavító műtétekről

ÖSSZEFOGLALÁS:

A szerző' a Pécsi Fül-Orr-Gégeklinikán 1967-1982 között gyermekkorúakon (4-14 év) végzett hallásjavító műtétéinek eredményeiről számol be. 26 beteg 29 fülén 41 műtét történt.
Négy fülön mesotympanális, 13 fülön cholesteatomás krónikus otitis állt fenn. Nyolc fülön az incudo-stapediális ízület traumás eredetű sérülését, 4 fülön stapedectomiát igénylő dobüregi fejlődési rendellenességet talált.
A műtéteket előző közleményeiben leírt technikával végezte.
A műtét előtt aktív suppuratio állott fenn 16 fülön. A gyógyulás sikeres volt 16, azaz az összes esetben.
Dobhártya-rekonstrukció történt 18 fülön (17 kron. otitis és 1 basisfracturához csatlakozó incus-diszlokácíó esetében). Normális novomembrán alakult ki 13 esetben, korrekció után további 2 esetben. Három esetben lateralisatio, Ш. részleges letapadás áll fenn. (Egy betegünknél ventillációs tubus biztosítja a dobhártya korrekt helyzetét.) A dobhártyarekonstrukció tehát primeren sikeres 13, korrekció után 2, összesen 15 esetben a 18 közül.
Cholesteatoma miatt nyitott technika alkalmaztatott 8, zárt technika 5 esetben. II. szakaszban a középfül revíziója történt 8 esetben, melynek során 5 rezlduális cholesteatoma (hámgyöngy) fordult elő, kivétel nélkül a dobüregben. Második revízió 2 alkalommal történt, reziduális cholesteatoma nem volt található.
Kísérlet történt a hallás javítására 25 fülön. Négy fülön columellisatio még nem történt.
A legutóbbi ellenőrző vizsgálat szerint jelentős és tartós hallásjavulás lépett fel 18 fülön, 1 esetben a műtét előtt jó hallás változatlanul jó maradt. Második, korrigáló műtét után tartósan jó a hallás 2 esetben, így tartós és jelentős hallásjavulást sikerült elérni 21 esetben 25 közül.
A hallásjavítás sikertelen 4 fülön a 25 közül.
Változatlanul rossz maradt a hallás műtét után 1 fülön és 1 fülön jelentős, de átmeneti javulás után. Két fülön romlott vezetéses jelleggel. Jelentős, percepciós jellegű hallásromlás vagy megsüketülés nem fordult elő.
A fenti adatok kedvezőknek mondhatók.
Az idült gennyedés megszüntetésének esélye tympanoplasztikával legalább olyan jó, mint a „klasszikus” műtétekkel, ezért a konzervatív kezeléssel dacoló mesotympanális otitis esetén várakozni nem érdemes, míg cholesteatoma esetén nem is ajánlatos.
Mesotympanális otitisek esetén az exacerbatióktól való félelem nem tűnik megalapozottnak.
Cholesteatoma esetén zárt technika végezhető, ha kétszakaszos műtétet tervezünk. Fel kell készülnünk azonban a cholesteatoma szokatlan kiterjedésére, ami miatt a zárt technika nem alkalmazható. Ilyen eseteknél jogosan merül fel a kongenitális cholesteatoma gyanúja. A gyógyulás végett a dobüreg ilyen esetekben is lefedendő.
Technikai szempontból a műtét kétségtelenül annál könnyebb, minél idősebb a gyermek, ezért konzervatív kezelésre reagáló mesotympanális otitisek és gyulladással nem járó, vezetéses halláscsökkenést okozó dobüregi kóros állapotok esetén, a műtéttel várakozhatunk. Ezen esetekben figyelembe veendő a másik fül állapota. Kétoldali, nagyfokú halláscsökkenés sürgeti legalább az egyik fül hallásának - az adott helyzetnek megfelelő technikával végzett — mielőbbi javítását, kivéve az olyan eseteket, amikor a beavatkozás nagy valószínűséggel faciális bénulást okozhatna.
Nyitott kérdés jelenleg, hogy a fenestra ovalis agenesist érdemes-e fiatal korban korrigálni.

 

Operation for the improvement of hearing in children.

SUMMARY

The author reports on the results of operations performed at the Department of Otorhinolaryngology of the University of Medicine of Pécs between 1967-1982 on 4—14-year-old children to improve their hearing. 41 operations were carried out on 29 ears of 26 patients. Chronic suppurative otitis media was in the case of 4 ears and chronic otitis with cholesteatoma in that of 13 ears. Traumatogenic injury of the incudostapedial joint was discovered in 8 ears and tympanic malformation requiring stapedectomy in 4 ears. The operations were performed with a technique described by the author in a previous communication. Active suppuration existed in 16 ears before the operation. Curation of the disease was successful in 16 i.e. in all cases. Tympanic reconstruction was performed on 18 ears. (In 17 chronic otitis cases and 1 incus-dislocation associated with basal fracture). Normal novomembrane formed in 13 cases and after correction in further 2 cases. Lateralization, partial adhesion was found in 3 cases. (In one patient ventillation tube ensures the correct position of the tympanic membrane). Tympanic reconstruction was successfull primarily in 13 cases, after correction in 2 cases altogether in 15 of 18 cases. Because of cholesteatoma „open” technique has been applied in 8 cases and „closed” technique in 5 cases. In second stage the revision of the middle ear took place in 8 cases, in the course of which 5-residual cholesteatomas (epithelial pearl) were observed always in the tympanic cavity. Second revision occured on 2 occasions, no residual cholesteatoma was found. To improve the hearing attempts were made in the case of 25 ears. Columellisation has not been performed on 4 ears. According to the possible latest check examination significant and permanent hearing improvement was observed in 18 ears and ín 1 case the good hearing before the operation remained unchanged. In 2 cases the hearing was permamently good after the second corrective operation, thus permanent and significant hearing improvement was reached in 21 of 25 cases. Unsuccessful was the attempt to improve the hearing in 4 cases. Invariably bad remained the hearing ín 1 ear and in another after a significant though transitory improvement. The hearing of 2 ears showed worsening of conductive character. Significantly impaired hearing of perceptive character or deafness did not occur. The above mentioned data can be considered as favourable. The chance of curing the disease through tympanoplasty is at least as good as with „classic” operation. Thus to wait in case of rhronic suppurative otitis media resisting the conservative treatment is not worth while and in the case of cholesteatoma even not advisable. In case of chronic suppurative otitis the fear of exacerbation does not seem to be established. „Closed” technique can be used in the case of cholesteatoma if the operation is planned in two phases. However one must be prepared to the unusual extension of the cholesteatoma owing to this „closed” technique cannot be used. In such cases the suspicion of congenital cholesteatoma is well-grounded. In order of curation the tympanic cavity has to be covered also in such cases. From technical viewpoint the operation is undoubtedly easier if the child is older therefore in case of suppurative otitis media reacting to conservative treatment and in cases of pathological conditions of the tympanic cavity causing conductive hearing defects without inflammation the operation can be postponed. In these cases the condition of the other ear has to be taken into consideration. Bilateral serious hearing defect urges the earliest possible improvement of hearing of at least one ear- performed with a technique suitable for the given condition except in those cases where the intervention may cause facial paralysis with high probability. Whether or not the correction of fenestra ovalis agenesis is worth to perform in young age is still an open question.
