ÖSSZEFOGLALÁS:
Otosclerosis miatt stapedectomiával eltávolított stapes scanning elektronmikroszkópos vizsgálatát végezték öt esetben. Az otosclerotikus elváltozások a stapes talpon súlyos fokban, a szár és fejecs területén alig mutatkoztak. A stapes talp perilymphatikus felszínén az otospongiosus állománnyal összeköttetésben lévő' kráterszerű lacunákat mutattak ki, melyeken keresztül vörösvértestek, histiocyták és sejtlebomlási termékek juthatnak a perilymphába. Feltételezik, hogy ezek a belső fül krónikus toxikus károsodását hozhatják létre. A „stapedectomia vagy stapedotomia? ” kérdésben állást foglalnak a korai stape- dectomia mellett.

Data on the pathomechanism of the otosclerotic damage of the inner ear Scanning electro- microscopic examination of the stapes removed because of otosclerosis by means of stapedectomy has been performed by the authors in 5 cases. The otosclerotic changes appeared on the stapes base in severe grade, but in the area of the pedicle and the head only scarcely. On the perilymphatic surface of the stapes base crater-like lacunae connected with the otospongious substance were found, through which erythrocytes, histiocytes and cytolytic products may enter into the perilymph. It is presumed by the authors that these latter may elicit the chronic toxic damage of the inner-ear. As to the problem: „Stapedectomy or stapedotomy? ” the authors’ point of view in favour of the early stapedectomy is outlined.
