A gyermekkori periorbitalis gyulladások rhinologiai vonatkozásai

ÖSSZEFOGLALÁS:

Osztályuk 10 éves beteganyagának 62 periorbitalis gyulladás esetét dolgozták fel a szerzők. Ebből 38 extranasalis, 24 pedig rhinogen eredetű volt, mely utóbbi az esetek 39%-ának felel meg. Feltűnő azonban, hogy az orbitalis gyulladás képét mutató legsúlyosabb esetek 92%-ában orr- és orrmelléküreg eredetet találtak. Saját statisztikai adataikban 10 betegnél szerepelt sinusitis maxillaris, 2 betegnél ethmoidalis és 10-nél együttes eredet. Kihangsúlyozzák azonban az ethmoidalis eredet pathogenitását, mindegyik műtétre került (súlyosabb) esetnél szerepelt, míg a sinusitis maxillaris eredet a konzervatív kezelés lehetőségét biztosította.
Anyagukban feltűnő, hogy az aránylag enyhébb megjelenésű eseteknél (cellulitis orbitalis) a fiatalabb korosztályoknál — 3 éves kor alatt — találkoztak, a súlyosabb (phlegmone orbitalis) kórképekkel a 4—8 éveseknél, míg a legsúlyosabb esetek (abscessus retrobulbaris) a pubertás körüli években fordultak elő. Részletesen foglalkoznak szerzők a pathogenesissel, klinikummal, diagnosticá- val és therapiával. Rámutatnak, hogy a konzervatív therapia mérlegelése közben mindig gondoljunk az időbeni aktív sebészi beavatkozás (ethmoidecto- mia) fontosságára.

 

Rhinological bearings of the periorbital inflammations in the childhood.

SUMMARY

From the 10 years’ material of the authors’ department 62 cases of periorbital inflammations are reviewed by the authors. The inflammation was in 38 cases of extranasal — and in 24 oases of rhinogenous origin, — this latter corresponds to 39% of the total of the observed cases. As interesting finding it is noted, that in 92% of the most severe cases, showing the picture of orbital inflammation, nasal and paranasal sinus-origin was found. In the authors’ statistical data in 10 patients the inflammation was due to maxillary sinusitis, in 2 patients to ethmoidal sinusitis and in 10 patients simultaneously to maxillary and ethmoidal sinusitis. The pathogenecity of the ethmoidal origin is pointed out, seen in all operated cases; on the other hand, if the inflammation was due to maxillary sinusitis, mostly conservative treatment was possible.
It is worthy to note in the authors’ material that the relatively mild cases (orbital cellulitis) were observed in the patients under 3 years, the more severe forms (orbital phlegmon) occurred in the patients between 4—8 years of age, and the most serious cases (retrobulbar abscess) have been observed in the puberty.
The pathogenesis, the clinical features, the diagnosis and the therapy are discussed in detail. Even during conservative treatment the necessity of active surgical intervention, in due time, is to be taken into consideration.
