ÖSSZEFOGLALÁS:

A gége és garat sérülései ritkák. Oka ennek a relatív kicsinység ; háborús cselekményi célpontnak a gége kicsi, azonkívül védi az előre álló alsó állkapocs, a magas kabát és köpenygallér. A régi háborúban nagyon kevés volt az észlelésre került gégesérültek száma. Az állóharcok, ahol csak a fej van szabadon ezt a számot megnövelték. Az elmúlt háborúban a gége és garat sérültek közül a legtöbb meghalt vagy olyan súlyos volt az állapota, hogy rokkanttá vált. Gégesérülések esetén a nem gégész szakorvosok általában csak a fulladás és elvérzés veszélyének elhárítására törekszenek, de nem gyógyítják meg véglegesen a sérült gégét. Ezek az ellátásbeli hiányosságok teszik indokolttá, hogy a gége és garat sérüléseinek elsődleges végleges ellátásával foglalkozzunk.

 

Injuries to the Larynx and the Pharynx.

SUMMARY

The author reports on 14 cases of external and 7 cases of internal injuries to the larynx and the pharynx. There were two contusions, and a fracture of the larynx, three cut wounds, two stab wounds, three acute and three chronic gunshot injuries among the external cases. Among the internal ones there were two cases of foreign body injuries, two injuries inward the larynx caused by drinking hot alcohol, one injury caused by inhaling chlorine gas, and one by drinking lye. The treatment of patients with such injuries should be determined by the quality of the damage suffered by them. Definite treatment of the wound must be aimed at because the patient may be saved in this way from a lot of complications and a series of surgical interventions. The edema and hemorrhage in patients with contusions of the larynx got absorbed on conservative treatment. In cases of fracture of the larynx the fractured thyroid cartilage was fitted together by thyreotomy. The incised and stab wounds were treated by tracheotomy and then revision of the wound and stratified wound-closure was made. In cases of acute gunshot wounds the treatment consisted of tracheotomy, exposure of the larynx, removal of the bruised parts, suture of the mucous membrane and stratified wound closure. In cases of chronic gunshot wounds the larynú was exposed, the scars narrowing down the lumen of the larynx were extirpated, the defects of the mucousa with epithelial flaps covered and setting a plug regularly in the larynx stratified wound closure was made. Three weeks after operation the patients were decanulated. Among the internal injuries the situation was as follows. In the two cases of injuries caused by foreign boides healed on treatment for inflammation. In the cases of injuries to the larynx arising after intubation and tracheoscopy tracheotomy was to be made. The patients injűried by drinking hot alkohol and inhaling chlorine gas healed on conservative treatment. In the case caused by drinking lye the scars filling up the inside of the larynx were removed, then forming iaryngostomy and setting Tiersch’ flaps into the cavity of the larynx the laryngeal cavity was adequately shaped and the wound was closed later on by plastic surgery.
