Preoperative Preparations and Operative Indications with Infants from the Point of View Otology and Laryngology

SUMMARY

In view of the vaso-, thermo- and hydrolability of infants the increased sensitiveness is one of the most characteristic features of this age. The infant can adapt himself only by degress to the environmental conditions chiefly in the first six month. Chirurgical intervention beeing always a trauma it is a trauma in infancy all the more. For this reason infants should be operated on only in cases with vital indication and even then as far as possible only after careful pediatric conditional preparation. Narcosis must be of the shortest duration possible. The considerable loss of water occuring in consequence of emesis after narcosis must be made up. While the surgical solution of antrum-empyema has generally speaking of a life saving value taking in consideration other ontogenetical, topographic and immunbiological circumstances other laryngological and otologic operatins ought to be avoided because there are practically no closed paranasal sinuses at this age. In the place of the antrum of Highmore the limeless epithelial tubercles of the permanent teeth may be seen. Below the milk teeth-germs take place ready to eruption and giving X-ray shadows. The maxillary sinus developes gradually from the doubled up epithelium of the middle tract of the nasal canal in accordance with the progress of the eruption of the permanent teeth during the dentition. In consequence of the suppuration of the dental germs of the deciduous teeth a swelling may occur spreading over the root of nose and over the lower lid. The latter may be mistaken all the more because catarrhal disposition is one of the most characteristic featuresof early babyhood.
