ÖSSZEFOGLALÁS
A saccus endolymphaticus tumor egy rendkívül ritka daganat, ami előfordulhat sporadikusan vagy von Hippel-Lindau-(vHL) -betegséghez társultan. Egy sporadikus saccus endolymphaticus tumorban szenvedő beteg esetét ismertetjük a nemzetközi szakirodalom áttekintésével. A 44 éves nőbetegnél I I éve jobb oldali fülzúgás, forgó jellegű szédülés lépett fel, később progresszív, idegi jellegű halláscsökkenés, fél évvel felvételét megelőzően jobb oldalon perifériás arcidegbénulás alakult ki. MR- és CT-vizsgálatok a jobb sziklacsontban növekedést mutató, érdús képletet igazoltak, ami a belső hallójáratba tört és a hátsó koponyagödör felé terjedt. A radiológiai kép alapján felmerült saccus endolymphaticus tumor lehetősége, így a betegnél subtotalis petrosectomia, az oticus capsula teljes eltávolítása és nenzus facialis rekonstrukciója történt suralis grafted. A szövettani vizsgálat saccus endolymphaticus tumort véleményezett. Genetikai vizsgálat során a vHL gén területén nem találtunk mutációt. A beteg hallásrehabilitációja csontba horgonyzott implantátum (BAHA® Attract) behelyezésével egy évvel a tumoreltávolítást követően megtörtént.A saccus endolympaticus tumor ritkaságából adódóan jelenleg nincs egységes megállapodás sem a terápiát, sem a betegek utánkövetését illetően. Esetismertetésünk célja, hogy rámutassunk a saccus endolymphaticus tumor diagnózis felállításának nehézségeire, kihangsúlyozva a subtotalis petrosectomia fülsebészeti jelentőségét, valamint a hallásrehabilitáció fontosságát.
Endolymphatic sac tumor Case report and review of the literature.
SUMMARY
Endolymphatic sac tumor (ELST) is a very rare neoplasm whichmay occur sporadically or as part of von Hippel-Lindau (vHL) disease. We report a sporadic case of endolymphatic sac tumorwith an overview of the literature. A 44-year-old woman presented with I I-year history of right sided tinnitus and vertigo,later right sided progressive sensorineural hearing loss. Sixmonths prior to her admission total facial nerve paralysis wasrecorded on the right side. MR and CT examinations demonstrated a slowly growing, hypervascular tumor mass destroyingthe temporal bone on the right side, extending to the internalauditory meatus and posterior cranial fossa. Radiological findingssuspected ELST, so the patient underwent subtotal petrosecto-my plus removal of the otic capsule with facial nerve reconstruction with sural nerve. Histological examination demonstrated endolymphatic sac tumor. Genetic testing for mutations of VHLwas performed with negative result. The patient has undergonehearing rehabilitation with bone-anchored hearing aid (BAHA'Attarct).Endolymphatic sac tumors are very rare laesions. As a consequence at present, there is no consensus regarding the therapyand follow up of these patients. Our goal is to present the difficulties the diagnosis of ELST and draw the attention to the importance of subtotal petrosectomy and/or simultaneous or resulting hearing rehabilitation.
