ÖSSZEFOGLALÁS
Akusztikai szempontból kívánatos volna a protézis mediális vége és a stapestalp közötti merev összeköttetés kialakítása a stapes szuperstruktúrájának hiánya esetén. A glass ionomer cement ideális ragasztóanyagnak tűnik a protéziseknek a stapes talpához való fixálására. Tanulmányunkban célul tűztük ki egy olyan állatkísérletes modell kidolgozását, amelynek során vizsgálhatjuk a stapestalpra felvitt ionomer cementből kioldódó alumínium-ionok koncentrációjának esetleges változását a perilymphában. A vizsgált, összesen 25 Pannon fehér nyulat 3 csoportra osztottuk. 5 állatnál (I. csoport) a fülműtétnél nem használtunk glass ionomer cementet, 15 nyúlnál (II. csoport) cementet alkalmaztunk a stapes talpára és 5 állatnál (III. csoport) a stapestalpra történő cementfelvitel után megnyitottuk a vestibulumot, Mintát vettünk a vestibulumból ürülő folyadékból és meghatároztuk az alumínium szintjét. Az alumíniumszint nem szignifikánsan volt alacsonyabb all. csoporttól vett folyadékmintákban az I. csoportéhoz képest, és szignifikánsan volt kisebb az I. és II. csoportban a III. csoporttól vett mintákhoz képest. A glass ionomer cement a stapes talpának közelében használva kiváló ragasztóanyag és amennyiben perilympha szivárgással nem kell számolnunk, akkor az alumínium intoxikáció veszélye elhanyagolható. Nyitott vestibulum esetén a cement stapestalpra való felhelyezése ellenjavallt.
Application of ionomer cement onto the stapedial footplate: impact on the perilymphatic aluminum level. An experimental study on animals.
SUMMARY
From an acoustic aspect, fixation of the medial end of an ossicular replacement prosthesis to the stapedial footplate would bedesirable. Technically, glass ionomer cement seems an ideal material for this purpose. The objective was to determine the aluminum level of the perilymph after the application of ionomercement on the stapedial footplate in an experimental study onrabbits. A total of 25 Pannon White rabbits were divided intothree groups. Five rabbits (group I) underwent sham operation;in 15 animals (group II) ionomer cement was applied onto thestapedial footplate; and in 5 cases (group III) the application ofthe cement onto the footplate was followed by opening of thevestibulum. In groups of five, the animals were killed on day 1,7,30, 180 or 365 postoperatively. Fluid samples were taken fromthe vestibulum and their aluminum levels were determined. Theaverage aluminum level in the fluid was insignificantly lower ingroup II than in group I, but significantly lower in groups I and IIthan in group III. As a glue, ionomer cement safely can be applied onto the footplate without the threat of raising the perilymphatic aluminum level, provided that there is no perilymphleakage. However, in the event of an open vestibulum, the application of cement onto the footplate is to be strongly discourageddue to the danger of a consequent increase in the aluminum level in the perilymph and the cerebrospinal fluid.
