A cone-beam CT diagnosztikus értéke a szövettanilag igazolt otosclerosis eseteiben

ÖSSZEFOGLALÁS

Retrospektív tanulmányunk a cone-beam komputertomográf (CBCT) szerepét mutatja be az otosclerosis preoperatív diagnosztikájában. A tanulmányba 32, egyoldali stapedectomián átesett beteg került szövettanilag igazolt stapedialis otosclerosissal. A műtétek előtt minden esetben sziklacsont CBCT készült. A vizsgálat során 0,3 mm-es szeletvastagsággal és többsíkú rekonstrukcióval dolgoztunk, és az eltávolított stapestalpákát minden esetben szövettani vizsgálatra küldtük. A CBCT-n látott elváltozásokat a Marshall-féle osztályozási rendszernek megfelelően értelmeztük (0-3 grádus). A szövettani leleteket a több síkban rekonstruált CBCT képekhez viszonyítottuk. A szövettanilag igazolt aktív otoscleroticus gócokat a CBCT minden esetben azonosította (n = 2l), 100% szenzitivitással. Az inaktív otoscleroticus gócokat azonban a CBCT nem mutatta ki (n = I I, szenzitivitás = 0%). A CBCT vizsgálattal csak stapestalpat érintő elváltozásokat találtunk, retrofenestrális góc nem mutatkozott. Minden igazolt eset a Marshall szerinti I. grádusnak felelt meg a stapestalp elülső pólusán elhelyezkedő góccal. A CBCT megbízható képalkotó módszer az otosclerosis preoperatív diagnosztikájában, a nagyfelbontású CT-hez (HRCT) képest lényegesen alacsonyabb sugárterheléssel. Eredményeink a CBCT magas szen- zitivitását és specificitását igazolták az aktív otosclerosis hypodenz lézióinak kimutatása során.

 

Diagnostic value of cone-beam CT in histologically confirmed otosclerosis

SUMMARY

This retrospective case review was performed with the aim to asses the value of cone-beam computed tomography in the preoperative diagnosis of otosclerosis. A total of 32 patients with histologically confirmed stapedial otoslcerosis, who underwent unilateral stapedectomies were analyzed. Preoperative temporal bone scans were performed in ail cases. Cone-beam computed tomography imaging was characterized by a slice thickness of 0.3 mm and a multiplanar image reconstruction. Histopathologic examination of the removed stapes footlplates was performed in all cases. Findings of the cone-beam computed tomography were categorized according to Marshall’s grading system (from grade
0 to grade 3). Histopathologic results were correlated to multiplanar reconstructed cone-beam computed tomography scans, respectively. Histologically active foci of otosclerosis (n = 21) were idenified by cone-beam computed tomography in all cases with a sensitivity of 100%. However cone-beam computed tomography was unable to detect histologically inactive otosclerosis (n =11, sensitivity=0%). According to cone- beam computed tomography scans, no retrofenestral lesions were found and all positive cases were recruited into the grade
1 group indicating solely fenestral lesions at the anterior pole of stapes footlplates. In conclusion, cone-beam computed tomography is a reliable imaging method with considerably lower radiation dose than high-resolution computer tomography in the preoperative diagnosis of otosclerosis. These results indicate that cone-beam computed tomography has high sensitivity and specificity in the detection of hypodense lesions due to histologically active otosclerosis.
