ÖSSZEFOGLALÁS
Szerzők 63 éves férfi beteg esetét ismertetik, akinél három hónapja tartó gátolt orrlégzés, ismétlődő orrvérzések hátterében orrendoszkópia, majd CT-vizsgálat sinonasalis, azonos oldali orbitába terjedő térfoglaló folyamatot igazolt. Helyi érzéstelenítésben szövettani mintavétel történt, ami felvetette a malignus hemangiopericytoma lehetőségét. Tekintettel az orbita involváltságára (szemmozgások szabadok voltak, kettős látás nem jelentkezett), illetve a malignitás gyanújára, laterális rhinotomiából a bulbust megtartva radikális kimetszés történt. Posztoperatív immunhisztopatológiai vizsgálatok a hemangiopericytoma diagnózisát alátámasztották. A műtét óta eltelt 4 évben a beteg tumor-, illetve panaszmentes, távoli áttéte nem jelentkezett. Szerzők saját esetük kapcsán áttekintik az entitás hazai és nemzetközi irodalmát, a diagnosztikus folyamat lépéseit, és vázolják a terápiás lehetőségeket.
Malignant sinonasal haemangiopericytoma
SUMMARY
We present a case report of a 63 years-old man, who presented a three-months history of unilateral nasal obstruction and epistaxis. ENT examination and endoscopy showed a large soft purple mass in the left nasal fossa, reaching the nasoharynx. CT scan revealed wide extension to the ethmoid sinuses, and even in the orbita, but there was no bone destruction. The patient didn’t show any eye symptom. Biopsy was taken in local anesthesia. Histology raised the possibility of haemangiopericytoma. In view of the orbital spread and the histology the patient was operated via lateral rhinotomy approach and the tumor was completely removed. Four years after operation our patient is free of symptoms and recurrence, without any metastasis.
