ÖSSZEFOGLALÁS

A pajzsmirigy sebészetben sokáig egyeduralkodónak számítottak a nyitott feltárásból végzett beavatkozások, a minimál invazivitásra való törekvés az endoszkópos technika rohamos fejlődésének köszönhetően a sebészet e területét sem hagyta érintetlenül. A Miccoli nevével fémjelzett Minimál Invazív Video-Asszisztált Thyreoidectomia (MIVAT) napjaink egyik legdinamikusabban fejlődő műtéti technikája. Az eljárás során a jugulumban ejtett 2,5-3 cm-es metszésen keresztül endoszkópos kontroll mellett nyílik lehetőség a pajzsmirigy reszek- ciójára. A folyamatosan bővülő indikációs kör ellenére a módszernek egyelőre a benignus és limitált kiterjedésű papil- láris pajzsmirigy folyamatok terápiájában van létjogosultsága. Az eljárás fő előnyei a kiváló kozmetikai eredmény a gyorsabb sebgyógyulás és a rövidebb kórházi tartózkodás. A módszernek az endoszkóposán reszekálhatatlan méretű lebenyek, a lokálisan előrehaladott malignus folyamatok és bizonyos szövettani típusok, illetve az eszközpark ára szabnak határt.
A szerzők a PTE KK Fül-Orr-Gégészeti és Fej-Nyaksebészeti Klinikán szerzett kezdeti tapasztalatok bemutatásán keresztül részletezik a MIVAT technikát, indikációs körét, előnyeit, nem megfeledkezve az eljárás hátrányairól sem.

 

Minimally Invasive Video-Assisted Thyroidectomy (MIVAT) - first steps at the Department of Otorhinolaryngology, Head and Neck Surgery of Clinical Centre of the University of Pécs

SUMMARY

External approach is considered to be the gold standard for thyroid surgery, however the developement of endoscopic technique and the strive for minimal invasive approaches has become widespread in thyroid surgery. Minimally Invasive Video-Assisted Thyroidectomy (MIVAT) hallmarked by the name of Miccoli is one of the most dinamically developing surgical technique. During the procedure resection of the thyroid gland is available via a small incision with the length of 2,5-3 cm made above the sternal notch under endoscopic control. Despite increasing number of indications the method plays a role only in the treatment of benign lesions and low-grade papillary malignancies with limited extent for the present. Main advantages of the technique are excellent cosmetic results, faster wound healing and shorter hospital stay. The method is delimited by locally advanced malignancies, certain histological types of thyroid cancers, endoscopically unresectable lobe size and the relatively high cost of instruments.
Based on the initial experiences gained at the Department of Otorhinolaryngology, Head and Neck Surgery of Clinical Centre of the University of Pécs, the authors describe the surgical steps, indications and contraindications of MIVAT along with the advantages and disadvantages of the technique.
