Kísérletek a stapes superstructurájának pótlására

ÖSSZEFOGLALÁS:

A szerzők két betegükben a hallócsont láncot olyan „talp-dobhártya” autogen corticalis columel- Iával pótolták, ami két részből áll. A lateralis rész a dobhártyát érinti és feltételezésük szerint azzal összenő. A medialis rész medialis végét az első esetükben a talphoz, a második esetükben a mellső szár maradványához rögzítették ionomer cementtel. A medialis columella a stapes su- perstructurája pótlásának tekinthető. A medialis és lateralis columellák nem lettek egymáshoz rögzítve, csupán érintették egymást, így megközelítőleg imitálni lehetett az incudo-malleolaris ízület functióját.
Ezen megoldások audiológiai teljesítménye meglepően jónak bizonyult. Az első fül esetében a csont-lég rés a 0,5-4 kHz-en mért értékek átlagával számolva 6,6 dB 7 évvel a műtét után. A második fül esetében a csont-lég rés záródott 2 évvel a műtét után. A műtét előtti idegi komponensek nem változtak.
A stapes superstructurájának hiánya esetén alkalmazandó columelláknak a talphoz rögzítése a hallócsontlánc recon- structio régi, de megoldatlan problémája. A szerzők remélik, hogy a leírt módszerek megoldást jelenthetnek. Tárgyalják az ionomer cement lehetséges toxicitásának kérdését.

 

SUMMARY

The authors have reconstructed the ossicular chain with autogenous bone “drum to footplate” columella consisting of two parts in two patients. The lateral part touches the tympanic membrane and is expected to grow together with the membrane. The medial end of the medial part has been fixed to the footplate in the first ear, in the second ear to the remnant of the anterior crus with ionomer cement. The medial part can be regarded as the substitute of the superstructure of the stapes. The two parts have not been fixed together; they are only touching each other, imitating the function of the incudo-malleolar joint.
The audiological efficiency of these solutions has proved to be surprisingly good. 7 years after the operation the air- bone gap is 6.6 dB (as an average of the values obtained at 0.5-4 kHz) in the first ear. In the second ear the air- bone gap has been closed 2 years after the operation. The preoperative sensory-neural components have not changed.
The fixation of the columella to the footplate in the absence of stapes superstructure represents an old, but unsolved problem of the ossicular reconstruction. The authors think the described methods may offer a possible solution. The question of the possible toxicity of the ionomer cement has been discussed.
