A craniofacialis resectio szerepe a rosszindulatú orrmelléküreg-daganatok sebészi kezelésében

ÖSSZEFOGLALÁS:

Napjainkban az ethmoid sejtrendszer malignus daganatainak sebészi kezelésében a craniofacialis resectio egy jól megalapozott, a hagyományos (maxillecto- mia, rhinotomia lateralis) műtéti megoldásokkal szemben számos előnyt jelentő megoldást képvisel. Az operációt követően az 5 éves túlélési adatok a hagyományos műtéti eredményekkel összehasonlítva kedvezőbbek, a műtét kiváló kozmetikai eredménnyel, alacsony morbiditással végezhető, s palliáció céljára is alkalmas.
Szerzők közleményükben egy 53 éves férfibetegük kapcsán a bal orrfél felső harmadában észlelt, a lamina cribrosat is érintő intestinalis típusú adenocarcinoma sikeresen operált esetét ismertetik. Részletezik a műtét előtti kivizsgálás szempontjait és ismertetik az általuk fontosnak vélt műtéti előkészületi lépéseket. Részletesen bemutatják a craniofacialis resectio lépéseit, ismertetik a műtét indikációjának és ellenjavallatának szempontjait, s felhívják a figyelmet az esetleges komplikációk lehetőségére is. Három évvel a szövődménymentes műtétet követően betegük tumormentes. Az irodalmi adatok áttekintése kapcsán, illetve más munkacsoportok kedvező tapasztalatait is figyelembe véve a szerzők megállapítják, hogy a craniofacialis resectio létjogosultságot nyert az ethmoid sejtrendszert érintő daganatok sebészi kezelésében.

SUMMARY

At present craniofacial resection represents a well-defined procedure in the management of malignant maxillo-ethmoidal tumors. Based on the philosophy of "en block" resection, this procedure has several advantages compared to the conventional surgical techniques (maxil- lectomy, lateral rhinotomy). Five-year survival data seem to be more favorable, the cosmetic results are excellent, the morbidity is low and the procedure can be carried out for palliation too.
The aim of this paper is to describe the case of a 53 year old male patient diagnosed with an intestinal type adenocarcinoma affecting the upper third of the left nostril. The tumor, which reached the cribriform plate, was successfully resected by carrying out a craniofacial resection.
The authors describe not only all the steps of craniofacial resection in detail, but highlight the aspects including indications and contraindications of this procedure and underline the possible postoperative complications. The patient underwent a complication-free procedure and 3 years following the operation is free from recurrence. From the review of the medical literature and on the basis of experiences of other institutes the authors conclude that craniofacial resection is a very useful surgical tool in the management of malignant maxillo-ethmoidal tumors.
