Vertebrobasilaris keringési elégtelenség okozta cochlearis és vestibularis működészavarok

ÖSSZEFOGLALÁS:

A vertebrobasilaris terület ischaemiája a centrális és periphériás vestibularis változások széles skáláját hozhatja létre. Az egész cochleovestibularis rendszert a vertebrobasilaris érrendszer látja el. Amikor a cochleovestibularis rendszer vascularis károsodásáról beszélünk, a diagnózis egyéb betegségek kizárásán alapul. Mivel a cochleovestibularis rendszer artériái semmilyen eljárással nem tehetők láthatóvá, az orvos a vascularis eredetre csak következtetni tud, főleg a rizikófaktorokból és a más érterületen kimutatható vascularis elváltozásokból. A szerzők 65 szédülő, ép vagy nem ép hallású cereb- rovascularis beteg adatait dolgozták fel. A betegek életkora 45 és 64 év között volt. A betegeken teljes otoneu- rológiai, audiológiai vizsgálat és BERA történt. A vascularis betegség kórisméje a Doppler , az MRI és MRA vizsgálatokon alapult. A vestibularis vizsgálat során különböző centrális és perifériás laesiók voltak kimutathatóak. Ha a vizsgálatok egyéb betegséget kizárnak, ha a betegnek vascularis rizikófaktorai vannak, és más, a képalkotó vizsgálatok számára hozzáférhető területen vascularis károsodás igazolható, akkor a beteg cochleovestibularis működészavarát keringési eredetűnek kell tartani.

SUMMARY

Vertebrobasilar ischaemia often can be responsible for a wide range of central and peripheral vestibular syndromes. The whole cochleovestibular system is supplied from the vertebrobasilar system. When the vascular disorder of the cochleovestibular system is mentioned, the diagnosis is based on the exclusion of other diseases. Since the arteries of the cochleovestibu- lar system cannot be directly visualized, physicians must deduce from the vascular risk factors and the vascular lesion of other territories the origins of the vascular cochleovestibular disease. The authors examined 65 vertiginous patients with or without hearing loss, in whom the cause of the vertigo and hearing loss was diagnosed to be vascular disease. The age of the patient-group was planned to be between 45-64- Complete neurootological and audiological examination, and ABR were performed. The diagnosis of the vascular disease were based on carotid and vertebral artery Doppler sonography, MRI and MRA. The vestibular examination showed several lesions of the central and peripheral vestibular system. In most of the patients multiple risk factors of cerebrovascular disorder can be observed. If the examinations exclude other diseases, if the patients have vascular risk factors and if other territories of brain accessible for imaging methods show vascular disorders, the cochleovestibular system disorder may be considered to be of vascular origin.
