Egyszerű és biztonságos primer és secunder punkciós módszer hangprotézis implantációhoz

ÖSSZEFOGLALÁS:

Szerző a hangprotézis implantációhoz egyszerű és biztonságos punkciós technikát dolgozott ki. A garatot feltárja a larnygoscoppal, melyet egészen a nyelőcső bemenetig vezet. A szerző által konstruált, majd a német Richard Wolf cég által sorozatgyártásra továbbfejlesztett endoextralaryngeális tűátnyomó műszer disztális végét a laryn-goscopon keresztül bevezeti a nyelőcsőbe. A műszert addig vezeti, míg annak disztális, hajlított, tompa vége nem tapintható a tracheostoma felső harmadában. A tűt a 2/0-s prolen fonallal belülről kifelé átnyomja a tracheostoma felső harmadában. A hegyes fémkúpon - melybe egy tű van rögzítve - és a katéteren átvezeti a hurkot képező kettős dróthuzalt és azt az átfúrt ellentartó golyó mögött rögzíti. Ezt követően a 2/0-s prolen vezető fonalat összeköti a drót hurokkal. A fonal és a dróthuzal meghúzásakor a fémkúp hegyes vége — belsejében a tűvel — perforáfja a lágyrészeket és magával húzza a katétert. Ezután a hangprotézis könnyen behelyezhető a szokásos módon a fisztulába. Az oesophagotrachealis (ET) punkció előnye a Blom-Singer-punkcióval szemben, hogy a nyelőcső vagy a garat hátsó falának sérülése kizárt és a punkcióval kapcsolatos szövődményekkel nem kell számolni.

SUMMARY

The author introduces his technique developed for a simple endo-extra-oesophagotracheal puncture instead of the Blom-Singer method. The Blom-Singer puncture was a pioneer method but in some cases it was difficult to perform. The author performs his simplified puncture-technique by forming the fistula with the help of his endo-extralaryngeal needle carrier (developed for mass production by the R. WOLF Co.) and a pointed metal cone with a needle built in fixed to the catheter. The operation is performed under JET anaesthesia that makes the patient free from any pains and leaves enough room for the surgeon in the area of the stoma. Through the laryngoscope the distal end of the endo-extralaryngeal needle carrier is led into the esophagus. The instrument is pushed forward as long as its distal bent end is palpable from the outside in the upper third of the tracheostoma. The needle with the thread (Prolen 2/0) is pushed through from inside out in the upper third of the tracheostoma. A double wire forming a loop is led through the pointed metal cone (in which there is a needle built in) and the catheter, and it is tied behind a counterfixing pierced ball. The Prolen 2/0 leading thread is then knotted with the wire. By pulling the thread and the wire, the pointed end of the metal cone with the needle built in perforates the sorf parts and pults the catheter with itself. After the fistula has been prepared for the implantation of the prosthesis, the metal cone, the thread and the wire are cut off the end of the catheter. The advantage of this endo-extra-esophagotracheal puncture technique is that the risk of injuring the backwall of the esophagus or pharynx is eliminated.
