Idegi vagy érzékhámsejt eredetű a percepciós nagyothallás otosclerosisban?

ÖSSZEFOGLALÁS:

Szerzők a SOTE Fül-Orr-Gégeklinikán 1988 óta utólag műtéttel igazolt egyoldali, kevert típusú nagyothalló otosclerosisos betegeken (n=16) a recruitment-jelenség ki- mutathatóságát elemezték Fowler szerint, alternáló kétoldali hangosságegyensúly- próbával. A betegcsoportban 9 esetben teljes recruitment, 7 esetben részleges recruitment volt kimutatható. Meglepő módon a betegek csontvezetési küszöbének Browning és Gatehouse szerinti korrekciója a stapes fíxatios fülön az ép fülekhez képest jelentéktelen küszöb-különbséget eredményezett. Mindez felveti:
1. recmitment-jelenséget előidéző Corti-szerv- (külső szőrsejt-) károsodás kialakulását otosclerosisban, a belsőfül-funkciót korrekten tükröző csontvezetési hallásküszöb már az ép fülhöz képest normális értékeinél is.
2. A Fowler-teszt diagnosztikus értékének kritikusabb szemléletét olyan esetekben, amelyekben jelentős mértékű vezetéses nagyothallása is van a betegnek (mint otosclerosisban).
Mindezek alapján véleményünk szerint, a részleges recruit ment-jelenség az otosclerosisos kevert típusú nagyothallásban nem kóijelző Cortiszerv-lézióra. Az 1000 Hz-en mért recruitmenttel járó esetek azonban az otosclerosis Cortiszerv-károsító hatása mellett szólnak.

SUMMARY

The authors analyse the revealabihty of recruitment phenomenon accordingly to Fowler by alternative bilateral loudness balance testing in patients (n=16) having unilateral otosclerosis with mixed type hearing loss and, have been operated since 1988 in the E. N. Т.-Clinic of the Semmelweis Medical University Budapest.
In this patient-group there was to demonstrate, that in 9 cases there has been total recruitment, till in 7 cases was only partial recruitment to observe. Surprisingly, the correction of bone-conduction threshold according to Browing and Gatehouse, has resulted in insignificant threshold-difference in otosclerotic ears compared to normal ones. All this raise the question:
1. Is there also a development of damage of Corti-organ (outer hair cells) causing recruitment in otosclerosis at normal values in bone-conduction threshold, reflected correctly the function of inner ear, compared with the normal ear?
2. Is it necessaiy to evaluate the diagnostical worth of Fowler-test more critically in such cases, in which the patient has a conductive hearing loss of significant degree (like otosclerosis)?
On the basis of mentioned, in opinion of the authors, the partial recruitment phenomenon in otosclerotic mixed type hearing loss is not a pathological sign to lesion of the Corti-organ. On the other hand, cases of recruitment measured on 1000 Hz, speak for Corti-organ’s damage in otosclerosis.
