ÖSSZEFOGLALÁS:
441 otosclerosis miatt végzett interpositio (stapedectomia) műtét kapcsán az arcideg vonatkozásait vizsgálja. 1 eset műtét közben fejlődési rendellenességnek bizonyult, mely az arcideg lefutásában is szokatlan helyzetet teremtett. A chorda tympanit a műtétek 25,6%-ában el kellett vágni, vagy elszakadt. Az arcideg csontos csatornájának dehiscentiái nagyobb nehézséget nem okoztak, viszont a mélyen fekvő Fallop csatorna akadályt jelentett a kengyel talpának teljes eltávolításában. 1,36%-ban jelentkezett interpositio után arcideg- hűdés, ezek reverzibilisek voltak, 1 esetben a homloki ág működészavara egy évnél hosszabb ideig tartott. Az arcideg műtéti feltárására szükség nem volt. Megemlíti a műtét előtti megfelelő tájékoztatás fontosságát.
SUMMARY
Importance of the facial nerve in otosclerosis operations.
In connection with 441 interposition operations (stapedectomy) performed because of otosclerosis the bearings of the facial nerve are examined by the author. During the operation one case proved to be developmental anomaly, which caused abnormal conditions and anomalous shape of the facial nerve. In 25,6% of the operations the chorda tympani was to be dissected, otherwise it was broken in two. The dehiscences of the bony canal of the facial nerve did not cause considerable difficulties, -on the other hand, the deep-seated Fallopian canal put obstacles in the total removal of the stape’ sole. After the interposition facial paralysis occurred in 1,36% of the cases, which proved to be reversible,— in one case, the functional disturbance of the frontal branch lasted longer than one year. Operative exposure of the facial nerve was not necessary. The importance of adequate preoperative orientation is stressed by the author.
