ÖSSZEFOGLALÁS:
A szerzők 250 microlaryngochirurgia kapcsán szerzett anaesthesiologiai tapasztalatukról és problémáikról számolnak be. Részletesen foglalkoznak külön a felnőtt- és külön a gyermekkori microlaryngoscopiáknál általuk alkalmazott anaesthesiás eljárásokkal. Nyomatékosan hangsúlyozzák a gégészeti vizsgálati módszerekben jártas anaesthesiologus előnyeit, és felhívják a figyelmet az adott speciális gégészeti problémákra, melyekkel az anaesthesiologus- nak számolnia kell.
Anesthesiological methods and problems in the micro-surgery of the larynx
SUMMARY
On the basis of 250 micro-laryngosurgical eases the anesthesiological experiences and the arisen problems are discussed by the authors. The methods of anesthesia used in the case of microlaryngoscopy in adults and in children are dealt with separately. In the case of general anesthesia in adults the premedication, the beginning and the duration, as well as the discontinuance of the narcosis are dealt with in detail. The risks of the use of Tonogen in the case of the use of halogenized inhalation narcotics are pointed out, and the pharmacologic properties of the drugs used by the authors are enlisted. The Gamma-hydroxibutiratnatrium — Gamma-OH — intravenous mononarcosis, used by the authors in children under 25 kg of body-weight , its advantages and inconveniences are discussed. The slow falling asleep, the irregularibility of the deepness of the sleep and the long after-sleep are mentioned as disadvantages. The age and sex distribution of the patients, as well as the method of intubation are summarized in tables. The beneficial role of the anesthesiologist expert with the laryngological examination methods is emphasized, and some special laryngological problems are pointed out, which are to be taken into consideration by the anesthesiologist.
