Kímélő radikális fülműtétek késői szanációs és funkcionális eredményei

ÖSSZEFOGLALÁS:

A tympanoplasticai szemlélet elterjedése, az operációs mikroszkóp és a micro- chirurgiai műszerek használata a radikális fülműtétek szanációs és funkcionális eredményeit is megváltoztatta. Osztályukon évek óta e szempontok szerinti kímélő radikális műtétet végeznek. 199 betegükön történt 207 fülmútét késői eredményeit tárgyalja a műtét előtti anatómiai lelet és hallás, valamint a műtét utáni állapot és hallás szempontjából. A kímélő technikával végzett radikális fülműtét szanációs és funkcionális eredményei jobbak, mint a hagyományos radikális műtéteké. Ezt a tényt a tympanoplastica indikációjakor és értékelésekor számításba kell venni. A kímélő radikális fülmütét a belsőfület kevésbé terheli, és megbízhatóan teljesíti ma is feladatát a szövődmények megelőzésében és gyógyításában. Kiterjedt ostitis, tartós otorrhoea nagy kiterjedésű cholesteatoma mellett első ülésben ez újabb technikával végzett kímélő radikális fülműtétet ajánlja.

 

Late sanatory and functional results of protective radical ear operations

SUMMARY

The increasing use of tympanoplasty, the operation microscope and the microsur- gical instruments lead to the change of the sanatory and functional results of the radical ear operations. On this basis, in the author’s department protective radical operations are performed since several years. In regard of the preoperative anatomical status and hearing, as well as of the postoperative status and hearing the late results obtained in 199 patients and after 207 ear operations are discussed by the author. The radical ear operations performed with protective technique give better sanatory and functional results than the traditional radical operations. This fact is to be taken into consideration at the indication of tympanoplasty. The protective radical ear operation signifies lesser inconvenience of the inner ear and it is more advantageous also in respect of the prevention and the treatment of complications. The protective radical ear operation performed with this new method is recommended by the author for the treatment of extended ostitis, lasting otorrhea with simultaneous cholesteatoma, to be carried out in the first sitting.
