ÖSSZEFOGLALÁS:
A szerző ismerteti a hangvezető rendszer reconstruction ára alkalmazott eljárását régebben végzett radicalis fülmütét üregében.
A dobhártya reconstructiójára temporalis fascia-lebenyt használ, melyet a radicalis üreg szabadon transplantált bőrével fed.
A hallócsont-láncolatot csont-autotransplantatumból formált columellá- val pótolja. Ép stapes esetén a columellát a stapes-fej és a dobhártya közé helyezi. Hacsak a stapes-talp található meg, egy hosszú, T alakú columellát helyez a dobhártya és a talp közé.
Ha a stapes vagy a talp fixált, stapedectomiát végez a dobhártya-reconst- ructio utáni teljes gyógyulás bekövetkezésekor és a columellát a dobhártya, valamint a fenestra oválist fedő fascia-lebeny közé helyezi.
A csontos dobgyűrű reconstructiójára, mely a jó functionalis eredmény elérése szempontjából fontos képletnek látszik, szintén csont-autotrans- plantatumot használ.
Reconstruction of the. sound conducting system in the cavity of radical ear intervention previously performed.
SUMMARY
The procedure of the author for the reconstruction of the sound conducting system in the cavity of previously performed ear surgery is presented.
For the reconstruction of the tympanic membrane a temporal fascia graft is used which is covered by the freely transplanted skin of the radical cavity.
The chain of the auditory ossicles is replaced with columella produced from the autograft. In case of intact stapes the columella is placed between the head of the stapes and the ear-drum. If only the foot plate of the stirrup can be found, a long T- shaped columella is placed between the foot-plate and the tympanic membrane.
If the stapes ot the foot plate is fixed, stapedectomy is performed at the time of total recovery after leconstruction of the ear-drum and the columella is placed between the ear-drum and the fascia covering the fenestra ovalis.
For the reconstruction of the osSeal ring of the tympanic membrane, which appears to be an important Substrate to achieve good functional results, also bone autografts are used.
